For those who will participate in an interview or discussion group, please read this Consent to Participate form before we meet. If you are able to print and sign this form you may do so, but I will also bring the form to our meeting.

In addition to participating in an interview or discussion, I ask that you complete a brief online socio-economic survey. I will send you a secure link to this survey over email.
